By accessing, browsing, or using this website, its directories, dynamic provider profile pages, search functions, and related features (collectively, the “Site”), you acknowledge that you have read, understood, and expressly agree to be bound by these Terms and Conditions (“Terms”). If you do not agree to these Terms, you must immediately discontinue use of the Site.​

• Hospice Guidebook is not a healthcare provider, medical referral service, or regulatory body.

• Content does not constitute medical advice, diagnosis, treatment recommendations, or legal endorsements.

• Legal, medical, or critical care decisions should never be made solely based on, without limitation, the layouts or data presented on this platform; always consult a qualified physician, healthcare professional, or family advisor

Data Sources, Historical Lag, and Accuracy

The Site aggregates CMS federal data alongside custom fields from subscribing agencies. Because CMS metrics update on delayed cycles, displayed data may be dated or out of sync with an agency's real-time footprint. Hospice Guidebook does not warrant the accuracy, timing, or reliability of any third-party information, federal data metrics, including, without limitation, CAHPS Star Ratings or HCI scores.

AI-Generated Content Notice

Hospice Guidebook does not use AI to generate, scan, or alter any provider directory data, descriptions, or profile content. Listing information is sourced directly from CMS public data or entered by the subscribing provider. AI is used only in two limited contexts: (1) Demonstration Images; AI-generated sample profile/directory images shown to prospective subscribers, not depicting any real agency; and (2) Blog/Educational Articles are occasionally drafted with AI assistance but independently sourced and human-reviewed before publication.

Directory Subscriptions, Badges, and Gated Features

• The Site operates a tiered directory model splitting raw historical federal data listings from premium paid placements (Standard and Premium Tiers).

• Badge Logic and Suppressions: Structural indicators like the Urgent Response Badge are conditionally gated based on active monthly subscription monetization layers. The presence or absence of a badge does not represent an official regulatory endorsement by CMS or Hospice Guidebook; it reflects a paid feature intersection layer tracking an agency's reported immediate intake availability.

• External Verification: Ranking sorts, premium badges, and custom copy block parameters are subject to change without notice. It is the user's responsibility to verify an agency’s active CMS Certification Number (CCN) status independently.
   

Reporting Errors and Corrections

• We strive to maintain an optimized, bug-free, and clean database environment. If you are a consumer, patient, family member, or hospice provider representative and identify factual inaccuracies, broken telephone routing links, incorrect zip code mapping arrays, or processing errors in the data layers, please report them to us.

• Upon receipt of a valid notice, we will investigate and apply necessary updates to our static or dynamic CMS collection layers in accordance with our administrative cycles.
   

Subscriber Content Standards

By claiming a listing and/or submitting content to a provider profile (“Subscriber Content”), the subscriber agrees to the following:

• Authorization to Claim Profile: subscriber warrants authorization to claim/manage the listing on the provider's behalf.

• Multiple claims for the same profile will require submission of specific information to prove eligibility submitted to: info@hospiceguidebook.com

• Accuracy and Currency: subscriber warrants content is accurate/current; HG does not independently verify it and disclaims liability for inaccurate content.

• Prohibited Content: no offensive/discriminatory material; no reference to any other provider/location; no URLs outside the designated field; no content unrelated to the listed location; no unlawful, fraudulent, defamatory, or misleading content; no disclosure of another individual's personal/health information.

• No Disparagement or Cross-Reference: each listing must describe only the single Medicare-certified location it represents.

• Contact/Link Information: URLs, phone numbers, and emails may only be submitted through designated subscription-tier fields.

• Review, Removal, Enforcement: HG may reject, edit, remove, or suspend content or a listing at its sole discretion, without notice, for violations. In the event of termination or suspension, refunds will not be issued to subscribers. 

• License to Display: subscriber grants a non-exclusive, royalty-free license to display/reproduce/format submitted content while the listing remains active.

Directory Subscriptions, Badges, and Error Reporting

The Site separates raw CMS listings from premium paid placements. Badge presence (e.g., Urgent Response Badge) does not represent an official CMS/Hospice Guidebook regulatory endorsement; rankings, badges, and copy parameters are subject to change, and users should independently verify an agency's active CCN status. Factual inaccuracies, broken links, or mapping/processing errors may be reported to info@hospiceguidebook.com; upon a valid notice we will investigate and update per our administrative cycles.

 

Billing, Subscriptions & Cancellations

Subscriptions (Basic, Standard, Premium) bill monthly or, where selected, annually (Standard Annual and Premium Annual). Both monthly and annual plans will auto-renew until canceled. Cancellation may occur at any time via the account dashboard; the subscription remains active through the already-paid period, with no refund or proration, for either monthly or annual plans. Pricing/features may be updated but will not affect an already-paid billing period.

Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, HOSPICE GUIDEBOOK AND ITS OPERATORS, DEVELOPERS, AND PARENT ENTITIES, HEIRS, SUCCESSORS AND ASSIGNS  SHALL NOT BE LIABLE FOR ANY DAMAGES ARISING FROM ACCESS TO, OR INABILITY TO ACCESS, THE SITE — INCLUDING RELIANCE ON PERFORMANCE INDICATORS, CONTACT METRICS, GEOLOCATION RESULTS, OR CARE CHOICES BASED ON WEBSITE CONTENT. TERMS MAY BE MODIFIED AT ANY TIME; CONTINUED USE AFTER A CHANGE INDICATES ACCEPTANCE OF THE UPDATED TERMS. 

TO THE FULLEST EXTENT PERMITTED BY LAW, HOSPICE GUIDEBOOK LLC SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, CONSEQUENTIAL, PUNITIVE, OR SPECIAL DAMAGES, INCLUDING LOST PROFITS, LOST BUSINESS OPPORTUNITIES, REPUTATIONAL HARM, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATING TO YOUR USE OF THE SITE.

IN NO EVENT SHALL  HOSPICE GUIDEBOOK LLC BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, RELIANCE, EXEMPLARY, PUNITIVE, OR CONSEQUENTIAL DAMAGES, INCLUDING , WITHOUT LIMITATION, LOSS OF PROFITS, REVENUE, DATA, GOODWILL, OR OTHER INTANGIBLE LOSSES, ARISING OUT OF OR RELATED TO YOUR USE OF OR INABILITY TO USE THE SERVICE, EVEN IF PERSPECTIVE PARTNERS HAS BEEN INFORMED OF THE POSSIBILITY OF SUCH DAMAGES.

EXCEPT FOR LIABILITY THAT CANNOT LEGALLY BE LIMITED, HOSPICE GUIDEBOOK LLC’S TOTAL CUMULATIVE LIABILITY SHALL NOT EXCEED THE PROFESSIONAL FEES PAID UNDER THE APPLICABLE ENGAGEMENT GIVING RISE TO THE CLAIM.

 

Class Action Waiver

You and Perspective Partners agree that any dispute resolution proceedings will be conducted only on an individual basis and not in a class, consolidated, or representative action. You waive any right to a jury trial and to participate in class action litigation.

Force majeure

Neither party shall be responsible for delays or failures caused by events beyond its reasonable control, including, without limitation, natural disasters, severe weather, governmental actions, labor disputes, supply chain issues, acts of terrorism, cyber incidents affecting critical infrastructure, utility failures, transportation disruptions, or other similar events.

Mutual Non-Disparagement

The Parties acknowledge that any disparaging comments by either Party about the other Party are likely to substantially depreciate the business or personal reputation of the Parties. The Parties agree to act in good faith so as not to harm the business or personal reputation of the other Party in any way. The Parties further agree not to directly or indirectly defame, disparage, or publicly criticize the services, business, integrity, veracity or reputation of the other Party, its owners, officers, directors, or employees in any forum directly or through any medium of communication. The Parties agree not to, without limitation, directly or indirectly, through executive officers or directors, defame, disparage, or publicly criticize the integrity, veracity or reputation of the other Party.

Modifications to Terms

We reserve the right to modify, rewrite, or update these Terms and Conditions at any time to reflect software engineering upgrades, new feature integrations, or regulatory adjustments. Continued use of the Site following any changes indicates your full acceptance of the updated terms.
